All providers who provide therapy or other care for the participants in the Georgia Physicians Health Program must go through our simple credentialing process. We take the care of our participants seriously and want to insure that all of our providers have active licenses as well as expertise and knowledge in the care of addiction, especially as it pertains to healthcare professionals.

To become credentialed by the Georgia PHP, you simply need to download the credentialing packet in the “downloads” section of this website. If you have questions, please not hesitate to contact us. Complete the credentialing information, and send it to us with the $25 credentialing fee. If you are not sure if you qualify, it may be in your best interest to contact us first, avoiding the work and the fee in applying if our network is not right for you.

All facilities to provide assessments or treatment for the participants in the Georgia Physicians Health Program must go through our simple credentialing process. We take the care of our participants seriously and want to insure that all of our facilities have the necessary licenses and expertise the care of addiction, especially as it pertains to healthcare professionals.

To become credentialed by the Georgia PHP, you simply need to download the credentialing packet in the “downloads” section of this website. There are two separate credentialing forms for Georgia PHP facilities. One applies to facilities that provide assessments, and a second for facilities who provide healthcare professional specific addiction treatment. If your facility performs both of these functions, you do need to fill out both applications. If you have questions, please not hesitate to contact us. Complete the credentialing information, and send it to us with the $75 credentialing fee. If you’re not sure if you qualify, it may be in your best interest to contact us first, avoiding the work and the fee in applying if our network is not right for you.
